Our person-centered approach puts you “in the driver’s seat” while we ride alongside to provide the amount of support that is needed maximizing your independence and dignity.
Program Overview
We provide 24-hour community-based Customized Living housing and services for adults living with mental Illness or other disability. We help individuals maintain independence while also being a part of the larger community.
Services include:
Registered Nurse Services
Behavior Support
24-hour staffing
Personal Care
Appointment management
Medication management
Meal preparation
Housekeeping
Socialization and activities
Qualifications
18 years of age or older
Diagnosis of a mental illness, brain injury, or other disability
Willing to adhere to care plans created by medical providers
Services are funded through the Waiver Programs
We accept private pay for individuals that don’t qualify for public funding
